Require candidates to submit following requirements in two (2) folders, (original and authenticated Xeroxed copies) prior to convening of training.

  • 1. Bio data (CSC Standard Format)
  • 2. Personal History Sheet (PHS)
  • 3. Physical Examination & ECG and Chest x-ray result
           *  Chest x-ray, ECG and laboratory exams which include CBC, Urinalysis,
                  and drug test should be availed from the PHILIPPINE HEART CENTER in
                  Quezon City.
  • 4. Neuro - Psychiatric screening
          * Neuro-Psychiatric (NP) Screening should be availed from the following
                assigned psychiatrist from the Faculty Medical Arts Building, PHILIPPINE
                GENERAL HOSPITAL in Taft Avenue, Manila.
                Please refer to the next tab for the assigned Psychiatrist for each applicant.
